<span style="font-size: 10pt;"><span style="letter-spacing: 0.102px; background-color: #fcfcfc; font-family: Georgia, serif; color: #333333;">Colorectal cancers does not exclusively present with the familiar symptoms of change in bowel habit, rectal bleeding, anaemia, weight loss and abdominal pain. Sometimes, colorectal tumours may infiltrate local structures and present in a different manner. We present an interesting case of a 76-year-old Caucasian lady with locally invasive transverse colon carcinoma who presented initially with gallbladder empyema and later with upper gastrointestinal (GI) bleeding. To the best of our knowledge, this is the first case reported in the literature who presented in this way.</span></span>
