520 _a<span style="font-size: 10pt;"><span style="color: #333333;">The patient presented at 41 weeks of pregnancy with spontaneous onset of labor and underwent vacuum delivery due to maternal exhaustion. The uterus was well contracted but following episiotomy repair, extremely heavy bleeding was noted from several tears in the edematous vaginal mucosa. The bleeding was not controlled by the application of figure-of-eight compression sutures and the patient was transferred to the operating room for examination under anesthesia. As she was a practicing Jehovah's Witness, she had earlier signed an advanced directive declining blood transfusion.</span></span>
